Application for Phase 2B Parent Volunteer
(For children born in 2 Jan 2021 to 1 Jan 2022, both dates inclusive)

Submission of Application for Parent Volunteer in Chua Chu Kang Primary is now open and will close on 28 Feb 2026 or once the school had reached the maximum number of applications, whichever is earlier.

The total hours of voluntary service may be shared by both parents.

Before filling in the form, please ensure you have digital copies of the following documents:

• Both parents’ identification cards

• Child’s Birth Certificate

• Exit Permit (for Singapore PR only)

Successful applicants will be notified via email 2 weeks after the closing date.

The decision of the school is final and the school is not obliged to provide any reason for unsuccessful application.
